Story Behind the Curve

FY20: The twice-monthly meetings are serving our families well, with different dates and times available to meet with families to better accommodate their schedules. With the COVID-19 Pandemic, our meetings moved to virtual meetings starting March 2020.

As most community events have been cancelled or moved to an online presence, an in-person presence at community events is not possible at this time. Will continue to attend local virtual events within the community to inform agency partners of the resource available to them within the Local Care Team for the families they work with. The LCT Coordinator is working on designing a virtual update and Technical Assistance that can be shared with agency partners, as we work through the COVID pandemic. Continue researching additional funding sources and apply for smaller grants to fund materials events, guest speakers, and trainings - for both professional partners, and the community as well. 

Partners

The Somerset County LCT is comprised of the required representatives from the following agencies: Dept of Social Services, Dept of Juvenile Services, Health Department, Somerset County Public Schools, Local Management Board, Dept of Rehabilitative Services, Developmental Disabilities Administration, and Core Service Agency. Additional members are representatives from Wraparound Maryland and Maryland Coalition for Families.

Program Summary

Under Maryland Code (Human Services §8-407) a Local Care Team shall: 

1) be a forum for: 

    a) families of children with intensive needs to receive assistance with the identification of individual needs and potential resources to meet identified                 needs; and 

    b) interagency discussions and problem solving for individual child and family needs and systemic needs; 

2) refer children and families to: 

    a) care management entities when appropriate; and 

    b) available local and community resources; 

3) provide training and technical assistance to local agency and community partners; 

4) identify and share resource development needs and communicate with the care management entity, local core service agencies, provider networks, local management boards, and other local care teams in surrounding jurisdictions; and 

5) discuss a request for a voluntary placement agreement for a child with a developmental disability or a mental illness under § 5-525 of the Family Law Article. 

Data Discussion

FY18 - referral rate was low due to the transfer from Multi-D meetings to LCT meetings. LCT Coordinator began in March/April 2018 and attended Multi-D meetings to train on meeting structure. The single referral that was received at the end of April 2018 was a Multi-D family that needed to be re-engaged with community resources. After initial intake with family, LCT met in May and June to keep the family engaged in community resources. Case closed in July 2018. Microsoft Word and Excel used to track referral.
