Community Health Workers

What We Do

A community health worker (CHW) is someone who has a deep understanding of the community because of shared backgrounds, cultures, and languages. The McLennan County Community Health Worker Initiative (MCCHWI) is a program where community health workers (CHWs) help connect people to existing health and social resources.

Who We Serve

As a pilot program, the CHWs work with community members that live in the 76704, 76705, 76706, and 76707 zip codes. If you'd like to get paired with your Community Health Worker, please call the CHW Coordinator, Paula Solano, via phone at 254-750-5631 or send an email to paulas@wacotx.gov.

How We Impact

Integrating CHWs in the delivery of care will begin to bridge the gap between the community and the health and social services available, leading to optimal health outcomes for residents in McLennan County. Through this program, CHWs will connect community members to the resources needed to achieve better overall health and quality of life.
