Story Behind the Curve

In SFY 2021, 24 of the 64 (38%) local health departments and districts (LHD) reported developing a community health improvement plan (CHIP) within the last five (5) years on the Local Health Annual Report. This figure has decreased over the past two years. The decrease may be due to some CHIPs falling outside the five year scope.  A CHIP is also a prerequisite for governmental public health jurisdictions to apply for public health accreditation through the national Public Health Accreditation Board (PHAB).  SFY 2013 was the first year that DPH began tracking this information on the Local Health Annual Report.  It should be noted that in 2021, DPH took an extra step to validate CHAs and CHIPs that were submitted by LHDs to ensure they met pre-determinded characteristics.  This improvement in survey methodology may explain the decrease from 2019.  The data are current as of December 2021.

What Works

A Community Health Improvement plan (CHIP) is a pre-requisite in order to apply for accreditation through the Public Health Accreditation Board (PHAB). PHAB defines a CHIP as "a long-term, systematic effort to address health problems on the basis of the results of assessment activities and the community health improvement process. This plan is used by health and other governmental education and human service agencies, in collaboration with community partners, to set priorities and coordinate and target resources.
