Increasing access to preventive health services means that Vermonters are receiving support, resources and care upstream, in order to reduce more costly and more serious health problems down the line.

This measure is a composite from the All Payer Claims Database; it is measured consistently across all insurers. The data displayed here includes breast cancer screening, cervical cancer screening and diabetes assessment rates together in one measure for the adult population. As Vermont moves forward with the Blueprint for Health program and its integrated patient centered medical home and community health team model of care, this data can be an indicator of our success.
