1. We are happy to report that we have exceeded our target value this reporting period. Our case managers have self-monitored assessment due dates independently in order to meet deadlines.

2.We are proud of the positive outcome though we did not meet our target goal. We suspect that one client who scored at risk for depression did not receive a referral to mental health support or treatment because she refused and did not accept the referral.

3. We are happy to report that we have met our target value this reporting period. We have one undocumented client referred to mental health treatment but unable to receive services because of their documentation status.

Davis Street Family Resource Center, Brighter Beginnings Mental Health Services, La Clinica De La Raza, Mental Wellness Team at MPCAH, and A Better Way

1. Ensure that all new enrolled clients have an EPDS done at intake, quality assurance team will continue to perform 2-month audits.

2. Program supervisor will continue to follow-up with staff during supervision to ensure all EPDS are turned in on or before the deadline.

2) Program supervisor provides support to staff that offer mental health information/resources to clients who score 10 or higher. Staff will continue to attend mental health trainings.

1. New staff will implement referral training that was provided when entering referrals in ECChange.

2. Quality assurance coordinator will conduct an internal audit in ECChange bi-annually to flag all clients who scored high on the EPDS before the team reviews the HVSF in-depth.

3. The quality assurance coordinator also reviews the referral tab to ensure needed referrals are made.

4. The quality assurance coordinator will assure that the referral has been completed and not left in progress.
