Notes on Methodology
The CDC measure is a hybrid HEDIS measure; it requires both administrative claims data and data abstracted during a medical record review. The DVHA completed a medical record review for this measure for the first time in 2014. We did not repeat the MRR process in 2016 - 2018 for this measure, but did pick it back up in 2019.

Story Behind the Curve
This composite measure, with 7 different rates including this one for Hemoglobin A1-c testing, looks at how well Vermont Medicaid cares for the common and serious chronic disease of diabetes. Diabetes is one of the most costly and highly prevalent chronic diseases in the U.S. Many complications, such as amputation, blindness and kidney failure can be prevented if detected and addressed in the early stages.
Vermont Medicaid has developed a care management program, the Vermont Chronic Care Initiative, that helps Medicaid beneficiaries with the most need, including those with diabetes, to better manage their conditions. Please see Partners and Strategies below for more information.
Rates prior to 2014 were collected through administrative claims only, so are less accurate than the rates reported in 2014 and after which were collected using hybrid (claims + medical record review) methodology.
The trend line above shows that Vermont Medicaid performs slightly below the national Medicaid 50th percentile benchmark for this measure.
