Story Behind the Curve
Since reopening in March 2022, referrals have increased as a consequence of our concerted efforts to promote services and maintain/expand our referral network. Previous experience indicates what seems to be a usual fluctuation in referrals that may be seasonal – likely related to variations in ED, hospital, and clinic utilization over the course of the year. Many of our referrals come from healthcare professionals working within the healthcare system and so it would make sense that our referrals reflect a fluctuating volume of high risk patients being seen at any given time.

What Works
OAHR seeks to support medically fragile clients who, because of inadequate psychosocial supports, are unable to manage their health conditions adequately putting them at risk for out of home placement and jeopardizing their quality-of-life. We outreach healthcare and community services providers to identify appropriate clients and support a steady stream of referrals that can be managed by our small team of nurses.
Action Plan
OAHR aims to hire a CHOW to assist with community partner outreach. Goal is to build on current outreach efforts to provide 2 in-service events per quarter and develop regular newsletter/email distributions in an effort to keep community partners informed of OAHR services.
