Story Behind the Curve
Contracting and onboarding with Community Based Care Management Entities (CB-CMEs) that provide this service has been slow. In addition, providers have had difficulty confirming Medi-Cal eligibility of prospective patients.
Maximum number of slots available per month is 750 slots for Tier 1 and Tier 2 Care Management Services.
Alameda Alliance for Health projects enrolling up to 380 enrollees and establish up to 20 CBCME sites by December 2018, potentially raising the per member per month (PMPM) number to 2,250 by the end of 2018.
New file format is being developed to retaining members if they did not meet the criteria for a short period of time, so we can continue their enrollment and allows for more accurate data.
More reasonable targets are still to be determined.
Partners
Alameda Alliance for Health and Anthem Blue Cross are contracted with AC Care Connect to administer the Care Management Bundle and oversee the clinics, called Community Based Care Management Entities (CB-CMEs), that perform this service. As of June 2018, 15 CB-CMEs have been brought online to provide services, including:

Anthem Blue Cross was contracted by AC Care Connect as another Care Management Bundle administrator to further increase enrollment beginning July 1, 2018.
Will be looking into ROOTS' practice to determine successful strategies for enrollment.
