Partners
Bay Area Legal Aid provides individual legal assistance to AC Care Connect enrolled individuals and engages with various partners throughout the coordinated system to prevent and end homelessness. These partners include Alameda County Health Care Services Agency, Alameda County Housing and Community Development, EveryOne Home, and other homeless and housing providers to coordinate client care and ensure clients receive the services and supports they need to achieve and maintain health and housing stability.
