- Subrecipient will improve identification of SASH participants with social isolation and or loneliness and plan for interventions that will reduce the incidence of participants who score as “lonely” on UCLA Loneliness Scale. Assessment rates will increase by 3% (from May 15th 2018 report %) for all participants as of May 15, 2019.
