In Year 2 we recognized the number the NDPP participants was decreasing. We held a training meeting with all lifestyle coaches in September 2016 to reinforce the program. We also began to reinforce the referral process within clinics by implementing policies.

Barriers/Justifications:
NDPP was launched in the Panhandle service area in 2012. In 2014, NDPP in the Panhandle hit peak saturation. In 2015, the Panhandle NDPP System received the Model Practice Award from NACCHO, and PPHD was the first organization in the state to be fully recognized by the CDC. Since then, the attendance to the program has entered a maintenance phase rather than a building phase.

Why Is This Important?
The purpose of this performance measure is to assess local health department efforts to increase the number of people enrolled in a CDC-recognized lifestyle change program. Clinical studies have demonstrated that participating in an evidence based lifestyle change program often prevents or delays the onset of type 2 diabetes among people with prediabetes or a high risk for type 2 diabetes. Note: NDPP was launched in the Panhandle service area in 2012. In 2014, NDPP in the Panhandle hit peak saturation. In 2015, the Panhandle NDPP System received the Model Practice Award from NACCHO, and PPHD was the first organization in the state to be fully recognized by the CDC. Since then, the attendance to the program has entered a maintenance phase rather than a building phase.
We do not charge for NDPP classes in the Panhandle, so no participants were enrolled using 1422 funds.
