Year 2:
We recognized in Year 2 that the number of participants being referred with blood work was decreasing. We hosted a meeting with all lifestyle coaches in September 2016 to strengthen the referral back to the clinic for participants that are eligible by risk assessment for screening, and are also strengthening referral relationships with clinics through introducing policies.

Barriers/Justifications:
NDPP was launched in the Panhandle service area in 2012. In 2014, NDPP in the Panhandle hit peak saturation. In 2015, the Panhandle NDPP System received the Model Practice Award from NACCHO, and PPHD was the first organization in the state to be fully recognized by the CDC. Since then, the attendance to the program has entered a maintenance phase rather than a building phase.

Why Is This Important?
The purpose of this performance measure is to assess local health department efforts to; 1) increase the number of health care providers referring people with prediabetes or at high risk for type 2 diabetes to a CDC recognized lifestyle change program, and 2) increase the number of participants who were referred by a health care provider.
One of the standards for a CDC recognition is that a minimum of 50% of a program’s participants must have had a recent (within the past year), documented, blood-based diagnostic test indicating they have prediabetes or a history of gestational diabetes mellitus (GDM), according to one of the following specifications:
a. Fasting plasma glucose of 100 to 125 mg/dl
b. Plasma glucose measured 2 hours after a 75gm glucose load of 140 to 199 mg/dl
c. A1c of 5.7 to 6.4
d. Clinically diagnosed GDM during a previous pregnancy (may be self-reported)
Note: NDPP was launched in the Panhandle service area in 2012. In 2014, NDPP in the Panhandle hit peak saturation. We do not charge for NDPP classes in the Panhandle, so no participants were enrolled using 1422 funds. The data above reflect NDPP participants who were eligible by blood work.
