Story Behind the Curve
Project baseline was that health hubs would exist at 4 local health departments and 2 Federally Qualified Health Centers (FQHCs). A target for success was set to increase by 5 FQHCs and 5 local health departments; and that by 2014 the project would include all FQHCs and 9-10 local health departments.
*As of summer 2015, contracts are established with all 7 Federally Qualified Health Centers and 9 Local Health Departments to operate Health Hubs. The program has almost achieved it's original goal. It is anticipated that by 2016 the program will expand to include an additional 4-5 local health departments.

- As of spring 2016, contracts are in place for 20 local health departments and 6 of the 7 FQHCs, and it is anticipated that these will continue into 2017.
- As of spring 2016, 26 out of 26 Community Health Hubs have completed environmental scans and community resource scans.
