This composite measure, with 7 different elements, looks at how well Vermont Medicaid cares for the common and serious chronic disease of diabetes. Diabetes is one of the most costly and highly prevalent chronic diseases in the U.S. Many complications, such as amputation, blindness and kidney failure can be prevented if detected and addressed in the early stages.

Vermont Medicaid has developed a care management program, the Vermont Chronic Care Initiative, that helps Medicaid beneficiaries with the most need, including those with diabetes, to better manage their conditions. Please see Partners and Action Plan below for more information.
