Story Behind the Curve
This composite measure, with 7 different rates, looks at how well Vermont Medicaid cares for the common and serious chronic disease of diabetes. Diabetes is one of the most costly and highly prevalent chronic diseases in the U.S. Many complications, such as amputation, blindness and kidney failure can be prevented if detected and addressed in the early stages.
Vermont Medicaid has developed a care management program, the Vermont Chronic Care Initiative, that helps Medicaid beneficiaries with the most need, including those with diabetes, to better manage their conditions. Please see Partners and Strategies below for more information.

Rates prior to 2014 were collected through administrative claims only, so are less accurate than the 2014 and 2015 rates which was collected using hybrid (claims + medical record review) methodology.
