Story Behind the Curve
As described in previous measure, interventions are designed to meet specific care plan goals. Ideally, all interventions are executed and goals are met. However, unforeseen circumstances arise, clients have varying commitments to taking action to address their goals, health crises intervene, priority shift depending on a client's changing situation and needs. As a result, a realistic goal of 75% was set – meaning that of 111 planned interventions, 83 of them would be completed. OAHR team came close to this target, completing 74 of the planned interventions this quarter. Completing this number of interventions is encouraging – indicating that the number and complexity of interventions was realistic and doable during the look back period of 6 months.
