OAHR nurses perform comprehensive biopsychosocial assessments and develop individualized care plans based on clinical priorities and client values.  Based on their assessment, nurses identify one TCM domain for which attaining the care plan goal is most likely to lead to improvements in quality-of-life.  In FY 22-23 we have consistently exceeded our 50% target rate for partially or fully meeting our clients' QOL goals.  For OAHR patient population, this is an impressive achievement since achieving these goals can be extremely challenging due to longstanding SDOH, medical and psychosocial complexity.
