OAHR nurses perform comprehensive biopsychosocial assessments and develop individualized care plans based on clinical priorities and client values. Based on their assessment, nurses identify one TCM domain for which attaining the care plan goal is most likely to lead to improvements in quality-of-life. In Q2, of 17 care plans that were up for review, 14 QOL goals were met or partially met, an 82% success rate. For OAHR patient population, this is an impressive achievement since achieving these goals can be extremely challenging due to client medical and psychosocial complexity. Multiple and long-standing SDOH barriers must be overcome in order to meet/partially meet QOL goals. This quarter OAHR was able to exceed the 50% target set for this measure.

Action Plan
Continue to track and collect data on the measure through the end of FY 22-23. At that time reevaluate value and progress. In general this is a very important measure since it is a key element necessary to achieve the OAHR mission and vision.
