Story Behind the Curve
The percentage of interventions that OAHR nurses were able to complete for cases in the denominator (either reassessed or closed during Q2) declined to 67% below the target of 75%. It is unclear at this juncture what factors interfered the most. As noted in the performance measures pertaining to care plan goals, OAHR nurses have found that services have been more difficult to access, systems more complicated to navigate, coordination with providers and communication with client/caregivers themselves more challenging. These barriers may account for why we were unable to reach our 75% target as anticipated. It is also possible that, since assessment is continual and occurs with each client contact, the nurses were able to refine their care plan goals and interventions and focus on those that were most likely to be impactful, while dispensing with those that were lower priority with lower anticipated impact.

No changes. Will continue collecting data and at the end of the FY, if still below target at that time will reevaluate and implement QI process tool such as PDSA to try to determine reasons we are not meeting target for completing interventions.
