OAHR nurse case managers created a combined 111 planned interventions as part of their individualized care plans.  The nurses plan interventions designed to meet their care plan goals for each client.  The number of interventions necessary to meet the goal is highly individualized and based on the client’s needs as well as the specific characteristics of each goal.  Thus, there is no target number for total interventions as this measure is highly variable and depends on the number of clients in the denominator (clients who were reassessed or whose cases were closed and, consequently, had care plan outcomes evaluated) and specific characteristics of each client’s goals.
