Story Behind the Curve

Of the total number of goals established to improve healthcare access and/or treatment plan adherence, what percentage were met or partially met? A goal of 75% reflects substantial success in achieving at least partial improvement in our clients' ability to access healthcare services and/or adhere to their treatment plans. Per this curve, OAHR nurses are below target for meeting care plan goals, at only 50% achievement.  This may be because the nurses, returning from pandemic work, may have ambitious goals for their clients and have not yet readjusted to the current healthcare and social services landscape – one in which services are more limited, more complicated to access, and clients are generally suffering from more social isolation.  OAHR team has found that, since reopening after COVID-19 deployments, services are more difficult to access, there are more barriers to provider communication, progress is more difficult and takes longer.  Additionally, communicating with families/caregivers/clients can be more challenging and require more frequent "touch" and persistence.

Action Plan

No changes.  Will continue collecting data and at the end of the FY, if still below target at that time will reevaluate and implement QI process tool such as PDSA to try to determine reasons we are not meeting target for meeting/partially meeting goals.
