Story Behind the Curve

Ambulatory sensitive conditions are diagnoses treatable in outpatient settings such as primary care. Hospitalization for ambulatory-care sensitive conditions is a measure of both quality and access to care and may indicate overuse of hospitals as a main source of care. Grafton County had a lower rate compared to the state in 2016 with 3202 and 3947 per 100,000, respectively. Rates from prior years cannot be easily compared because the source for data changed in 2019, the rate changed from 1,000 to 100,000 Medicare enrollees, and the definition of hospitalizations changed. Improving access to and quality of primary care and other outpatient services can help reduce hospital stays for ambulatory sensitive conditions.
