Story Behind the Curve
This measure examines the extent to which clinicians at Dartmouth-Hitchcock find and follow POLST (Physician Order for Life Sustaining Treatment) documents in a patient's medical record. POLST orders are physican orders completed with a patient experiencing severe health conditions and likley nearing end-of-life. These orders provide specific direction regarding the health care and emergency health care procedures a patient wishes for or does not wish for if they are unable to make decisions for themselves.
This data is collected through a death chart audit, reviewing medical charts of deceased patients to see of POLST documents were present, and if so, whether patient wishes were followed consistently by care team members. Dartmouth-Hitchcock aims to comply with these orders 100% of the time.
