Story Behind the Curve
This measure documents the percentage of patients who recieve their routine primary care (are 'medically homed') from Dartmouth-Hitchcock providers who have a completed Advance Directive scanned into their medical record. Advance Directives support and guide families and clinicians in making decisions when a patient is unable to make decisions for themselves. Dartmouth-Hitchcock works with patients to ensure such records are on-file in order to ensure that familes and clinicians understand a patient's wishes as they make decisions about care options.
