Story Behind the Curve
Rates of avoidable Emergency Department visits are an indicator of access to care as well as integration and coordination with community-based care systems. Dartmouth-Hitchcock and Cheshire Medical Center serve as the administrative host of the NH Region 1 Integrated Delivery Network, a collaboration among multiple health care and community organization providers to deliver better care for Medicaid beneficiaries with behavioral health needs. The Network aims to reduce avoidable ED visits through improved integration of behavioral health in health care and improvements in care transitions from hospitals to communities for persons who have behavioral health needs.
This data is not yet provided by the State of NH, but is a planned measure that will begin to be made available to Integrated Delivery Networks in the next year.
