The annual target percentage for cases terminated by mutual plan is 80%. During the first reporting period of FY21, 95% or 18 of 19 cases terminated by mutual plan. During the second reporting period of FY21, 100% or 12 of 12 cases terminated by mutual plan. A total of 97% or 30 of 31 cases terminated by mutual plan for FY21.
Definition: To “terminate services by mutual plan” means that both the clinician and client, or client’s parent/caregiver, agree that it is appropriate to end service at that time. A client’s failure to appear for appointments, resulting in service termination, is not considered termination by mutual plan. A client’s notification that he/she will be discontinuing service, when the clinician does not believe that sufficient progress has been achieved (e.g., treatment goals have been achieved) to warrant the termination of service, is not considered termination by mutual plan.
