Story Behind the Curve

Vermont has a decentralized system of adult inpatient care, where people in need of hospitalization are provided treatment at either the state-run inpatient facility, Vermont Psychiatric Care Hospital, or one of six Designated Hospitals throughout the state.
- Designated Hospitals provide treatment to both voluntary and involuntary patients.
- Hospitals with Level 1 Units include Brattleboro Retreat, Rutland Regional Medical Center, and Vermont Psychiatric Care Hospital.
Level 1 patients presenting for involuntary psychiatric admission who have severe psychiatric illness and require intense treatment services will be considered Level 1 patients eligible for inpatient bed day claims by the Department of Mental Health.
- An exception may be made for voluntary patients who require significant and more than usual resources, which means interventions as additional staffing on the unit, multiple or extended use of additional resources, including 1:1 and 2:1 staffing, extra psychiatrist or other clinical staff time, and repeated restraints or seclusions.
The Department of Mental Health Adult Care Management Team facilitates the coordination of admissions and aftercare services across the involuntary inpatient system.
- This team assists Designated Agency (DA) Emergency Service Teams and associated providers to triage individuals into programs for admission, as well as facilitating the referral process for individuals to step-down programs, transitional housing programs, and supportive housing units when they are ready to return to the community.
- To accomplish this task, the team works closely with hospitals by holding weekly clinical team meetings regarding inpatient status, supporting discharge and aftercare planning, creating a bridge to community programming, and providing technical assistance when necessary.
