Story Behind the Curve
By the conclusion of HFY2 of FY21, 100% or all 21 of the new families served during FY21 improved their Arizona Self Sufficiency Matrix (SSM) Housing score by at least 1 point (on the SSM scale from 1 to 5), with 18 families achieving a “safe” score of 3 (“In stable housing that is safe but only marginally adequate”), and 3 families achieving the “empowered” score of 5 (“Household is in safe, adequate, unsubsidized housing”). FSI case managers use the Arizona Self Sufficiency Matrix (ASSM) to help referred families identify their strengths and needs across multiple domains including housing, employment, health care, child care, family support, mobility, children’s education, adult education, parenting skills, safety, and more. The SSM is administered at program intake, every 3 months, and at program exit. The results of the ASSM are used by the case manager working in collaboration with the family to develop a plan that identifies measurable goals and outlines steps to achieve and track these goals.
